Analgesic action of extradural fentanyl.

the dial by two screws, but in the faulty vaporizer one had fallen out, into the top of the casing. With the cam loose, it was possible to turn the dial a few degrees clockwise beyond the OFF position, when a strong smell of enflurane was noticed in the fresh gas flow. Anaesthetists should be aware that unless the OFF mark on the concentration setting dial coincides with the datum mark, then their Enfluratec or, presumably, Fluotec 3, may still deliver vapour. I have reported this finding to Cyprane, making two suggestions. First, all existing Enfluratecs should be modified by fixing the cam to the dial by four instead of two screws. The cam and dial are already drilled and tapped for this, the extra holes being used at present to attach the name-plate to the dial. Second, the present design, with a hard steel spring working on a soft plastic cam, seems unsatisfactory and bound to cause excessive wear. The cam on the faulty vaporizer was badly worn after only 8 months' use. The rim of the concentration setting dial could be used as a cam, with different machining for different models, from the same stock of parts. A spring made of plastic in the same way as the one-piece hinges found on some plastic boxes would avoid the wear problem.

Patient-controlled extradural analgesia with bupivacaine, fentanyl, or a mixture of both, after Caesarean section.
In this randomized, double-blind study of 60 patients, we have assessed the analgesic efficacy of extradural bupivacaine and extradural fentanyl, either alone or in combination, after Caesarean section. Patients received 0.1% bupivacaine (group B), fentanyl 4 micrograms ml-1 (group F) or 0.05% bupivacaine combined with fentanyl 2 micrograms ml-1 (group BF) by patient-controlled extradural analg...

We have assessed the effect of combining extradural bupivacaine and fentanyl in 60 orthopaedic patients who received 0.125% bupivacaine (bupivacaine group), fentanyl 5 micrograms ml-1 (fentanyl group), or 0.125% bupivacaine combined with fentanyl 5 micrograms ml-1 (combined group), delivered by patient-controlled extradural analgesia for 24 h via a lumbar extradural catheter. Adding bupivacaine...
متن کاملExtradural analgesia with clonidine and fentanyl compared with 0.25% bupivacaine in the first stage of labour.
Conventional extradural analgesia during labour with 0.25-0.375% bupivacaine may induce motor weakness and subjective sensory deficit, reducing maternal satisfaction. Even in a regimen for ambulatory extradural analgesia (0.1% bupivacaine-fentanyl 2 micrograms ml-1), a potential for proprioreception impairment exists, which may impair safe ambulation. We have combined fentanyl with clonidine fo...

In a randomized, double-blind study of 39 mothers in labour, we have compared a loading dose of 0.5% bupivacaine 6.0 ml and fentanyl 100 micrograms given extradurally, followed by an infusion of 0.08% bupivacaine 15 ml h-1 plus fentanyl 37.5 micrograms h-1, with a loading dose of 0.5% bupivacaine 6.0 ml and saline 2.0 ml, followed by an extradural infusion of 0.08% bupivacaine alone, per hour. ...
متن کاملFentanyl and bupivacaine mixtures for extradural blockade.
In a prospective double-blind study, single dose lumbar extradural blockade was instituted in 80 healthy male patients undergoing lower abdominal surgery. Patients were assigned randomly to one of four groups. Each group received treatment with 0.5% bupivacaine 20 ml containing adrenaline 1:200 000. To this solution was added fentanyl 0, 50, 100 or 200 micrograms. The time to regression of anal...

We have compared the efficacy and side effects of extradural morphine with extradural fentanyl for postoperative pain relief. Thirty children (ages 1-16 yr) were allocated randomly to receive, after extradural administration of 0.5% bupivacaine 0.75 ml kg-1 and before surgical incision, extradural morphine 0.75 microgram kg-1 (group M), with an additional dose administered 24 h later or extradu...
